Referral Form
We welcome referrals from GPs, specialists, surgeons, practice nurses and allied health
professionals located in Melbourne Australia. Following referral, the patient will be contacted directly by Desanka Ogrizovic to
discuss their goals, explain the counselling process, complete the required intake forms and
arrange an appointment.
